Case Management Program Information

The goal of the Case Management Program for Frail Elders is to assist older persons to live in their own homes and delay (or avoid) institutionalization through home and community based services. A Case Manager will visit the client, assess their needs and coordinate needed services with local providers.

Services that are funded and/or provided directly by Seneca AAA are available on a contribution basis.
